Improving Health Outcomes

IHN-CCO’s Transformation Plan was approved by the Centers for Medicare and Medicaid in 2013. The plan helps to create programs and projects that improve local health care in Benton, Lincoln and Linn counties.
IHN-CCO and the Oregon Health Authority (OHA) work together to help achieve Oregon’s health system goals and encourage continuous quality improvement. It is recognized that transformation is a continuous process and that IHN-CCOs transformation plan will and should evolve over time, with the enduring goals of improving health outcomes, increasing member satisfaction and reducing overall costs.

IHN-CCO funds and supports pilots and projects that improve local health care in Benton, Lincoln and Linn counties. 

Learn About New Services:  Health-Related Social Needs 

Through Oregon’s 1115 Medicaid waiver, the state will be able to cover new services for Oregon Health Plan (OHP) members called health-related social needs (HRSNs). Community-based organizations and partners will be integral to this work and may be able to receive funding to help with these efforts.
