Child Abuse Prevention and Early Intervention Pilot

The Child Abuse Prevention and Early Intervention pilot, which ran from Jan. 2015 through Dec. 2016, increased collaboration between Family Tree Relief Nursery’s home-based program, InterCommunity Health Network Coordinated Care Organization (IHN-CCO) medical providers, and Oregon Department of Human Services Child Welfare.

Over the course of two months, the pilot served 102 families with 389 IHN-CCO members and linked 86 percent of children and 67 percent of adults to Patient-Centered Primary Care Homes. 

This pilot increased access and transition for high-risk IHN-CCO families, using a blended service model of Family Tree Relief Nursery’s home-based interventionist and traditional healthcare workers as an innovative way to help families. The pilot assisted by linking families to their medical home and family services, and providing stabilized support for improved outcomes for family health, stability, and attachment. 

Currently, Family Tree Relief Nursery serves an average of 477 members per month.
