Services for Members with Exceptional Needs

We want to make sure you are getting the care you need while taking care of complicated conditions. People with exceptional needs or chronic diseases may need extra care management. IHN-CCO can help.

Intensive Care Managers

IHN-CCO has Exceptional Needs Care Coordinators (ENCC). They help members who are older, disabled, or have other special needs. However, any IHN-CCO member may ask for these services. 

Intensive care coordination services include:

  • Helping members understand their benefits and the process for getting services or supplies
  • Coordinating health care with doctors, other providers and community agencies
  • Working with members who are having problems getting care
  • Finding other resources for members

Case Management Services

IHN also has case management services to help members who have chronic medical conditions (such as diabetes, asthma, or heart disease). Nurses and other care management staff can work with members, health care providers, or state caseworkers to find ways to meet the health care needs of these members.
